An 81-year-old man comes to the physician for a 2-day history of fever. He has history of hepatitis C and alcoholism. Leukocytes are 13,000/μL. Temperature is 102 F. Urinalysis shows 50 WBCs/hpf, 30 RBCs/hpf, and protein 100 mg/dL. A renal biopsy is shown.
